What it is

After reconstitution the vial holds a known concentration: the labeled amount of compound divided by the volume of diluent added. From that, any target amount corresponds to a specific volume to draw, which on an insulin-style syringe is read in 'units' (100 units = 1 mL on a U-100 syringe).

How it works

Illustrative method only: a 5 mg vial reconstituted with 2 mL of diluent is 2.5 mg/mL, i.e. 2,500 mcg/mL; on a U-100 syringe each 'unit' (0.01 mL) is therefore 25 mcg. The workflow is always the same — establish mg/mL, convert to mcg/mL, then divide the target by the per-unit amount.

What the evidence shows

This is standard dimensional analysis used throughout pharmacy practice; reputable calculators and pharmacy education materials apply the identical concentration-and-volume relationship. The errors seen in real reports are almost always unit confusion (mg vs mcg) or misread syringe graduations, not the algebra.
